Chronic pain in the foot and ankle can be caused by a variety of reasons.  The COASC offers the only board-certified orthopedic surgeon fellowship trained in Foot and Ankle on the peninsula.  Dr. Paul Maloof brings innovative treatment options to patients to address issues dealing with chronic pain and foot and ankle conditions.

Conditions

Bunion

Bunions affect the joint at the base of the big toe. It is a bony bump beneath the skin on the inner side of the foot. A bunion starts small, but over time it can grow to become very large. Bunions are more common in women. Pressure in the ball of the foot causes the bones to shift. This can be caused by high heels or shoes that are too tight or too narrow leading to arthritis in the foot.  Patients can benefit from more spacious shoes, pads or orthotic devices, medication, or ice. If these treatment options do not solve the problem surgery may be recommended.

Jones Fracture

A Jones fracture is a break in the 5th metatarsal and healing can be difficult.  It is located on the outer side of the foot.  This fracture can be caused by over use, repetitive stress, or a twisting injury.  In some cases, a Jones fractures can be treated by casting, but patients may need surgery to alleviate this condition. 

Procedures

Arthroscopy of the Ankle

Ankle arthroscopy is a minimally invasive procedure that orthopaedic surgeons use to diagnose and treat problems in the ankle joint. When arthroscopy is performed, it is usually due to ankle damage that is caused by arthritis versus a one-time traumatic injury. Two tiny incisions are made in the patient’s ankle where the arthroscopic camera and any necessary instruments can be inserted.

Bunyon Correction with Scarf and Akin Osteotomy

During bunion correction, portions of bone are removed, and the bones of the foot and toe are aligned properly to eliminate the bump on the outside of the foot.  The foot is placed in a special shoe and normal activities can usually be resumed in 6-8 weeks.

First MTP Joint Fusion (Locking Plate)

First MTP joint fusion treats severe arthritis of the joint at the base of the big toe.  Arthritis can cause pain and swelling and limit the ability to walk. A metal plate is implanted to prevent movement of the MTP joint.  Over time the bones will fuse. The patient will receive specific instructions from their doctor post-surgery to help the healing process.
